Completing the Alternate FAFSA: Scholars of Promise

This guide was created by the Boys & Girls Club TOPS College Success Staff in order to assist students in completion of the alternate FAFSA.

If you are unable to complete a FAFSA, you must submit an “Alternate FAFSA” in order to be considered for the Madison College Scholars of Promise Program. This guide will help ensure that those forms are completed as easily and accurately as possible! Please note that the alternate FAFSA for the 2018-2018 school year will use tax information from 2016.

Please reach out to your TOPS Coordinator, TOPS Enrollment Coach (Terrica), or TOPS Madison College Coach (Taylor) for additional support.

Forms can be turned in at the Truax Campus Welcome Center, emailed to financialaid@madisoncollege.edu, or faxed to (608) 243-4245.

Form title: Information breakdown: What will I need to fill this out? Tips:
Dependent Household Verification Form Who are your parents? What children do they have and provide support for (even if they don’t live with you)? Does anyone else live with you? N/A -Only list family members as in college if they are currently enrolled in college.
Parent Other Financial Information Form How much money do your parents have? What income did they receive in 2016? Did they receive any other benefits? Where did your parents work in 2016? Parents’ 2016 W-2 forms

-Rather than leaving columns blank, enter $0 so it is clear you did not miss any lines.

 

-Much of this information can be found directly on your parents tax forms or tax return transcripts.

Student Other Financial Information Form How much money do you currently have? What income did you receive in 2016? Did you receive any other benefits? Where did you work in 2016? Your 2016 W-2 forms

 

Your 2016 Tax Information

-Rather than leaving columns blank, enter $0 so it is clear you did not miss any lines.

Tax Filing Confirmation Form Did you work in 2016? Did you file taxes? Did your parents work in 2016? Did they file taxes? N/A All you need to do is check the appropriate box for yourself and parents, and follow the instructions on whether you will need to order a W-2 or Tax Return Transcript.
Parent & Student W-2’s from 2016 Yours or your parents 2016 W-2s. You should contact your employer if you do not have these. If you cannot get your W-2 from an employer, you can order from the IRS: https://www.irs.gov/help-resources/tools-faqs/faqs-for-individuals/frequently-asked-tax-questions-answers/irs-procedures/transcript-or-copy-of-form-w-2/transcript-or-copy-of-form-w-2.

Parent & Student IRS Tax Return Transcripts from 2016 Yours or your parents 2016 IRS tax return transcripts. You can order these for free here: https://www.irs.gov/individuals/get-transcript

 

(Choose “Get Transcript by Mail” to order for yourself or your parents)

Transcripts will take 5-10 business days to be mailed.

You can find the ITIN on yours/your parents tax forms. This is what you will use for identification instead of a SSN.

 

You need these if you/your parents did files taxes in 2016!
